Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing OT, per plan of care, with evidence-based techniques and in coordination with the health care team.
• Evaluates patient functional status and CMS documentation needs; consults with physician on the therapy plan of care.
• Observes, records, and reports patient response to treatment and changes in condition.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
• Function in home settings with patients of diverse ages, races, and conditions.
