Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for home health visits in the North Tucson area. This part-time or full-time role serves adult and geriatric clients in their homes across northern Tucson, AZ, collaborating with a multidisciplinary team to deliver rehabilitative care and skilled nursing to support independent living.

The OT will conduct evaluations, develop and adjust therapy plans, educate families, and document care using a state-of-the-art EMR

Qualifications

• Current Arizona Occupational Therapist license.
• Prior home care experience preferred but not required.
• Minimum one year of OT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive evaluations of functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Educate clients and families on rehabilitative care and activities.
• Document observations and interventions using a touchpad tablet.
