Job summary

Nurse on Call Home Care, LLC seeks a licensed Occupational Therapist to perform evaluations, develop and implement treatment plans, and guide a rehabilitation team in Fort Pierce, FL. You will assess patient needs, coordinate with physicians, and ensure care continuity.

The role includes creating home programs, training families, and documenting progress to maximize patient outcomes in a home-care setting.

Qualifications

• Bachelor of Science in Occupational Therapy from an accredited program.
• Successful completion of the National Certification Examination for Registered Occupational Therapist.

Responsibilities

• Performs evaluations and assessments and administers treatments and screenings.
• Develops treatment plans with clear goals and coordinates with physicians and team members.
• Establishes patient schedules and coordinates care with family and staff.
• Implements treatment plans, monitors progress, and adjusts as needed per physician orders.
• Creates home programs to maintain and improve patient function in daily environments.
• Documents assessment results, treatment progress, and termination of services.
• Trains patient, family, and staff in use of devices, orthotics, or prosthetics.
• Supervises and directs COTA in plan implementation.
• Reviews quality and appropriateness of services and aids in documentation and statistics.
