Job summary

Trilogy Home Healthcare, a Humana company, is hiring an Occupational Therapist (OT) to provide home health visits in Manatee County, FL on a PRN/per diem basis. The role focuses on helping patients regain mobility and independence, with collaboration across a clinical team and educators for safety and balance in the home setting.

The ideal candidate has an OT degree (AOTA-approved), Florida license, a valid driver's license, and at least 1 year of OT experience, with home health preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program (AOTA-approved).
• Active OT license in Florida.
• Valid Florida driver's license, active vehicle insurance, and clean MVR.
• 1+ years OT experience; Home Health experience preferred.

Responsibilities

• Deliver compassionate, high-quality home health OT care.
• Collaborate with doctors, Clinical Team Managers, and interdisziplinary teams.
• Help patients regain mobility, strength and independence after illness or surgery.
• Educate patients and families on balance, gait and fall prevention.
• Assess and ensure safe mobility in the home environment.
