Job summary

Concierge Home Care in Port Orange, FL is hiring a Home Health Occupational Therapist (OT) to provide 1:1 therapy in patients' homes with a flexible schedule.

You will evaluate patients, develop individualized treatment plans, supervise COTAs, coordinate with physicians, and help patients improve ADLs and mobility. PRN options, mileage reimbursement, and growth opportunities are available.

Qualifications

• Active, unencumbered Florida OT license.
• Valid driver's license and reliable transportation.
• CPR certification.
• Ability to supervise COTAs.
• Home health experience preferred, not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Therapy to improve ADLs, functional mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams; supervise COTAs.
