Adoration Health is seeking a Home Health Occupational Therapist to join our team in IL. The OT will provide 1:1 patient care, develop individualized treatment plans, and collaborate with a multidisciplinary team to enhance patient independence.

Qualifikationen

• Graduate of a CAPTE-approved Occupational Therapy program.
• Current OT licensure in the state and CPR certification.
• Two years of adult occupational therapy experience; home health preferred.
• Excellent observation and communication skills; problem solving and math skills.

Aufgaben

• Perform OT evaluation and treatment per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication with treatment team, physician, patient and family.
• Document services thoroughly for proper billing; supervise OT assistants as applicable.
