Qualifications

• Bachelor of Science or Master’s degree in Occupational Therapy.
• Licensed Occupational Therapist in the state of practice.
• Minimum of one year healthcare experience.
• Ability to drive ~20% of the day and hold a valid driver’s license.
• Experience with tablets, mobile devices and EMR software.

Responsibilities

• Collaborate within a team-based care model to manage and deliver patient care plans.
• Support coordination of home health OT services for patients in Itasca and surrounding areas.
• Maintain timely documentation and meet charting deadlines.
