You will select pay-per-visit or hourly arrangements, with mileage reimbursement and a company cell phone/tablet for documentation in Home Care Home Base EMR.

Qualifications

• OT degree and licensure required.
• BLS certification required.
• Proficient with documentation and record-keeping.
• Experience with EMR systems preferred.

Responsibilities

• Evaluate patient function and determine need for OT services.
• Implement and monitor individualized treatment plans.
• Document progress and communicate with care team timely.
• Educate patients/families and coordinate discharge planning.
• Communicate results to physicians, nurses and caregivers.
• supervise assistants and trainees as appropriate.
• Meet department productivity and process improvement standards.
