Job summary

A List Therapy Group is seeking a licensed Occupational Therapist (OT) in California to join its home health referral network as an independent contractor. Earn $95–$115 per completed visit with potential annual compensation of $150,000–$180,000 based on volume and eligibility, plus a $250 sign-on bonus and a $250 referral bonus.

Qualifications

• Active California OT license is required.
• Reliable transportation and valid driver’s license.
• Professional liability insurance.
• Commitment to high‑quality documentation and timely communication.
• Ability to work as an independent contractor.

Responsibilities

• Provide occupational therapy services in patients' homes per plan of care.
• Evaluate and intervene to support safety, independence and ADLs.
• Develop and update care plans aligned with physician orders and payer requirements.
• Assess home environments and recommend adaptations.
• Communicate with referral sources, physicians, patients and families as appropriate.
• Document progress accurately following payer and regulatory requirements.
