Job summary

AccentCare is seeking a Home Health Occupational Therapist for Firestone, Colorado. This full-time position covers a 40-hour work week with a Monday–Friday 8am–5pm schedule and no remote work.

You will provide OT services in patients' homes, coordinate with care teams, and document treatment plans. Salary ranges from $100,000 to $123,000 per year, plus a $15,000 bonus, reflecting an on-site, staff role in a growing home-health organization.

Qualifications

• Master’s degree in Occupational Therapy required
• Colorado OT license or eligibility required
• Minimum two years of home health experience preferred
• Must have valid driver’s license and ability to travel to patient homes

Responsibilities

• Provide home health occupational therapy services per plan of care
• Assess, diagnose, and treat patients to improve functional abilities
• Educate patients and caregivers on treatment plans and home safety
• Coordinate with nurses and other team members to optimize patient outcomes
