Job summary

Home Health Care 2000 is seeking an Office LPN to document patient encounters in real time during home visits or from dictated notes. You will support clinical staff by ensuring accurate EMR entries and billing-related documentation.

Responsibilities include managing office calls, coordinating care with the team, authorizing services for billing, and educating patients and families while maintaining clear communication and adherence to the plan of care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three years nursing experience.
• Community/home health or medical/surgical experience is preferred.
• Must be proficient in typing and good at spelling, punctuation, grammar, and oral communication.
• Must be able to listen to complex medical information and summarize clearly.
• Must possess excellent organizational skills.
• Must be proficient in medical diagnosis coding.

Responsibilities

• Responds to office calls related to clinical needs.
• Documents EMR and communicates with office staff clearly.
• Processes timely authorization of services for billing.
• Follows the plan of care and collaborates with the team.
• Provides clear communication and timely documentation of EMR.
• Assesses patient and family education needs.
• Coordinates home health services and reports needs to clinician.
