Job summary

Interim HealthCare, the nation’s first home care company, seeks a Home Health Occupational Therapist in Rochester, MN to deliver in-home therapy focused on independence and safe daily living. The role collaborates with an interdisciplinary team to implement personalized plans of care and monitor patient progress.

This full-time position offers a $7,000 sign-on bonus, competitive salary of $90,000–$100,000, mileage reimbursement, PTO, holidays, and comprehensive benefits, with growth and ongoing

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active OT license in Minnesota.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgement and strong interpersonal skills.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks at home.
• Work with a home health team to fulfill the plan of care and patient goals.
• Assess patient, set therapy goals, and document progress.
• Assist patients with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment and home safety improvements.
• Educate patient and family on plan of care and exercises.
