Job summary

Elite Home Health, part of LHC Group, seeks an Occupational Therapist for home health services nationwide. You will assess and address functional status, ADLs, fine motor skills and home adaptations within an evidence-based care plan.

Responsibilities include evaluating function, coordinating with physicians, documenting progress, and delivering personalized therapy in patients' homes. Current OT licensure and CPR required, valid driver’s license needed.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver’s license and access to reliable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care.
• Evaluates patient’s functional status and CMS documentation needs; consults with physician on therapy plan of care.
• Observes, records, and reports patient response to treatment and changes to condition.
• Conducts patient assessments evaluating functional ability using diagnostic and prognostic tests.
