Job summary

Mercy Home Health, a member of Trinity Health at Home, is seeking a Full-time Occupational Therapist based out of our Blue Bell/Montgomery County branch to support the Pottstown area.

The OT will consult, evaluate, plan and administer skilled therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury or surgery.

Qualifications

• Current PA licensure in Occupational Therapy.
• CPR certification required.
• Bachelor or Master Degree preferred.

Responsibilities

• Consults, evaluates, plans and administers skilled Occupational Therapy services for homebound patients.
• Collaborates with interdisciplinary team to meet patient needs and ensure quality care.
• Acts as patient care manager to oversee primary care delivery for a group of patients and maintain communication.
