Job summary

CenterWell Home Health is seeking a dedicated Occupational Therapist to work in the patient’s home. You will assess daily living skills, administer retraining programs, and collaborate with physicians and the care team to optimize patient function and independence.

In this role you will design treatments to restore strength and range of motion, document progress, and participate in care coordination and discharge planning. Supervisory duties for OT staff may apply.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Preferred: minimum six months of occupational therapy experience.
• Home Health experience preferred.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs with measurable objectives.
• Confer with physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, range of motion and function.
• Document patient care observations, interventions and evaluations.
• Coordinate care, discharge planning and act as a resource to the team.
• Supervise OT Assistant and Home Health Aide performance in OT services.
