Job summary

Hartford HealthCare at Home in Connecticut seeks a Visiting Occupational Therapist to evaluate patients and develop comprehensive OT care plans, coordinating with medical staff and caregivers in the patient home.

You will manage a patient caseload, supervise COTA and Home Health Aides, and ensure timely documentation and compliant billing while delivering compassionate, patient-centered care.

Qualifications

• Education: Bachelor's degree in Occupational Therapy.
• Connecticut license to practice Occupational Therapy.
• Experience: Three years' experience preferred.

Responsibilities

• Evaluates patient's level of function and develops appropriate OT care plan under physician direction.
• Coordinates patient care with Hospice team, nurses, therapists, social workers, and physicians.
• Delegates tasks to COTA and Home Health Aides and supervises their performance.
• Maintains accurate, timely clinical and reimbursement documentation.
• Manages daily patient caseload and ensures adherence to agency productivity standards.
