Home Health Certified Occupational Therapy Assistant

About the Role
Anuco Home Health Care LLC is seeking compassionate, dependable, and skilled Certified Occupational Therapy Assistants to provide occupational therapy services in patients’ homes under the direction and supervision of a licensed Occupational Therapist.
The ideal candidate has home health experience, strong communication and time-management skills, proficiency with Axxess Home Health EMR, and a commitment to providing excellent patient care and completing documentation within agency-required timeframes.
Responsibilities
- Provide occupational therapy treatments delegated and supervised by the Occupational Therapist.
- Follow physician orders and the written plan of care established by the supervising OT.
- Assist patients with therapeutic exercises, functional mobility, coordination, endurance, and activities of daily living.
- Provide ADL and IADL retraining, adaptive-equipment instruction, home-safety education, and caregiver training.
- Help patients improve their ability to bathe, dress, groom, toilet, prepare meals, and complete other daily activities.
- Reinforce home programs and safety strategies established by the Occupational Therapist.
- Observe and document the patient’s response to treatment and progress toward established goals.
- Promptly consult the supervising OT regarding changes in condition, treatment response, safety concerns, or recommended changes to the plan.
- Educate patients and caregivers regarding home programs, adaptive techniques, and safe performance of daily activities.
- Coordinate occupational therapy services with the supervising OT and interdisciplinary team.
- Promptly report falls, hospitalizations, incidents, complaints, and unmet needs.
- Participate in case conferences, staff meetings, peer review, QAPI activities, and required training.
- Protect patient confidentiality and comply with HIPAA and agency policies.
- Perform only those services for which the assistant has been trained, determined competent, and properly supervised.
Axxess and Documentation Expectations
- Complete every visit note accurately in Axxess within agency-required timeframes.
- Document treatment provided, patient response, progress toward goals, education, and care coordination.
- Ensure documentation is consistent with the physician’s orders and the OT-established plan of care.
- Report any recommended treatment changes to the supervising OT before altering the established plan.
- Complete progress notes and other assigned documentation promptly.
- Respond to QA corrections and return corrected documentation within the requested timeframe.
- Notify the office immediately if a clinical, scheduling, or technical issue may delay documentation.
- Understand that incomplete or late documentation may delay care coordination, authorization, billing, and future assignments.
Requirements
- Current, unrestricted Illinois Occupational Therapy Assistant license.
- Graduate of an accredited Occupational Therapy Assistant program.
- Current NBCOT certification as a COTA, when applicable.
- Home health experience strongly preferred.
- Axxess Home Health EMR experience preferred.
- Two years of occupational therapy experience preferred.
- Current CPR or BLS certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Professional liability insurance as required.
- Strong observation, communication, organization, and time-management skills.
- Ability to work independently while following OT supervision and the established plan of care.
- Ability to travel to patient homes and perform essential duties with or without reasonable accommodation.
Clinical Territories
Applicants may select one or more territories they can cover consistently.
West Territory:
Bellwood, Forest Park, Maywood, Westchester, Broadview, Stone Park, Berwyn, Chicago Ridge, Joliet, Lemont, Oak Lawn, Bridgeview, Hometown, Justice, Burbank, Palos Park, Palos Hills, Summit, Downers Grove, Burr Ridge, Lyons, and west/southwest Chicago.
Territory assignments depend on patient location, clinician availability, payer requirements, clinical competency, and agency needs.
Performance Expectations
Successful candidates will:

- Follow the established plan of care and supervising OT’s instructions.
- Complete assigned visits and documentation within required timeframes.
- Contact the office immediately regarding scheduling or patient-access problems.
- Report changes in condition and clinical concerns without delay.
- Maintain active licenses, certifications, and insurance.
- Respond promptly to agency communications and QA corrections.
- Demonstrate dependable attendance, sound judgment, and professional conduct.
- Provide compassionate, respectful, and patient-centered care.
