Qualifications

• Active Occupational Therapy licensure in state of practice
• Current CPR certification
• Valid driver's license with reliable vehicle or access to transportation
• Ability to work with diverse patient populations and home environments

Responsibilities

• Assess and evaluate patient care needs related to functional status and activities of daily living
• Evaluate patient function and coordinate with physician to develop therapy plan of care
• Document patient progress and communicate changes to the nurse supervisor and physician
• Perform home health OT assessments and adapt equipment as needed
