Optum, in partnership with LHC Group, is seeking an Occupational Therapist for Home Health in Louisiana. You will assess functional status, daily living activities, and home safety, and collaborate with the care team to plan and implement therapy services for patients in their homes.

The ideal candidate holds LA OT licensure, a valid driver's license, and demonstrates strong communication, independence, and documentation skills.

Qualifications

• Licensed OT in Louisiana (LA) required.
• Must have a valid driver's license and reliable transportation.

Responsabilités

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Conduct home health OT assessments and adapt equipment and plans of care.
• Collaborate with nursing and medical team to implement therapy plans.
• Document CMS-relevant findings and patient progress.
