Job summary

A compassionate healthcare provider in Firestone, Colorado is seeking an Occupational Therapist to join their team. Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid Colorado Occupational Therapist license.
• Valid BLS Certification.
• Experience in home health settings preferred.
• Exceptional communication skills and a patient-centered approach to care.
• Licensed driver with current auto insurance and good driving record.

Responsibilities

• Conduct occupational therapy assessments and evaluations.
• Develop and implement individualized treatment plans.
• Guide patients in self-care and activities for independence.
• Monitor patient progress and adjust care plans as needed.
• Build supportive relationships with patients and families.
• Collaborate with the care team for coordinated care.

The Role

We are seeking compassionate and skilled Occupational Therapists to join our team- PRN to Part-time opportunities available. You'll work with patients in the Firestone, Fredrick, Dacono, and surrounding areas, helping them regain mobility and improve their quality of life. This role is ideal for a therapist who enjoys building strong patient relationships and providing personalized, hands-on care.

What You'll Be Doing

• Conducting occupational therapy assessments and ongoing evaluations of patient's physical conditions and functional abilities using OASIS
• Developing and implementing individualized treatment plans tailored to each patient's needs
• Guiding and instructing patients in therapeutic self-care and activities directed toward improving independence and physical and cognitive functioning.
• Monitoring patient progress and making adjustments to care plans as needed
• Building supportive relationships with patients and their families, helping them through each step of their recovery journey
• Collaborating with the care team to ensure coordinated, comprehensive care
