Job summary

Caretenders, a part of LHC Group, is seeking an Occupational Therapist in Home Health to assess and address functional status, ADLs, fine motor coordination, home safety, and adaptive equipment as defined by medical conditions.

You will work with the health care team to develop plans of care, document CMS requirements, and deliver care in patients’ homes with emphasis on communication and independent practice.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates patient functional status and OT needs; consults with physician on plan of care.
• Observes and reports patient responses to treatment to nurse supervisor and physician.
• Conducts assessments using functional ability tests and CMS documentation.
• Provides OT services within state scope of practice and plan of care.
