Qualifications

• OT licensure in North Carolina is required.
• BLS certification required.
• Minimum 1 year of experience in home health or related setting.

Responsibilities

• Approximately 6 visits per day in home health.
• Perform patient assessments and develop care plans.
• Document care activities and progress in the EHR/Homecare Homebase system.
• Collaborate with interdisciplinary team to coordinate patient care.
