CONSUMER DIRECTED SERVICES INC. is seeking a Home Care Occupational Therapist to deliver person-centered therapy to adults in home settings, assess functional abilities, and develop individualized treatment plans.

You will implement evidence-based interventions to promote safety, independence, and quality of life, collaborate with an interdisciplinary team, educate patients and caregivers, and document outcomes to adjust plans as progress occurs.
