Job summary

Residential Home Health and Hospice is seeking a Home Health Occupational Therapist to provide skilled OT services in Clarkston, MI. You will deliver patient-centered care in patients’ homes, conduct ADL-focused evaluations, and develop individualized treatment plans to foster independence.

The role requires active OT licensure, at least 1 year of OT experience in home health, and a valid driver’s license with travel ~20% of the time.

What You’ll Do

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
