Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Occupational Therapist to deliver skilled OT services in patients' homes across Clinton Township and surrounding communities. You’ll assess ADLs, develop individualized treatment plans, and coach families on strategies to improve safety and independence.

Qualifications

• Active Occupational Therapist license in the state you work.
• Minimum of one year of OT clinical experience.
• Valid driver’s license and ability to travel ~20% of your workday.

Responsibilities

• Deliver skilled, patient-centered OT care in home health settings.
• Perform ADL-focused evaluations and develop individualized treatment plans.
• Educate patients and caregivers on adaptive techniques and home safety.
• Ensure compliant EMR documentation.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within assigned territory.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
