Job summary

Elara Caring is seeking a Home Health Occupational Therapist to provide one-on-one care in patients' homes across the Plymouth Territory. You’ll help patients rebuild essential skills, regain independence, and improve daily activities with personalized plans of care focused on safety and function.

Ideal candidates will have a degree in occupational therapy, be licensed, and have at least one year of clinical experience with the ability to travel up to 50%.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted Occupational Therapist license in the state of practice.
• At least one year of clinical care experience.
• Ability to travel throughout the assigned territory.
• Dependable vehicle, valid driver’s license, and current automobile insurance.
• Ability to travel up to 50%.

Responsibilities

• Evaluate patients’ functional abilities and rehabilitation needs
• Develop personalized plans of care focused on safety and independence
• Help patients improve daily activities such as dressing, bathing, cooking, and mobility
• Teach patients and caregivers how to safely use adaptive equipment
• Recommend equipment and home modifications that make daily life easier
• Monitor progress and adjust treatment plans as patient needs change
• Communicate important updates to physicians and other members of the care team
• Coordinate safe and effective discharge planning
• Provide clinical direction to Certified Occupational Therapy Assistants and other care team members

What You’ll Do

What You’ll Need

• Ability to travel up to 50%

• Flexibility to work varying or on-call hours when applicable

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
