- Develop and implement personalized care plans to improve daily living skills, independence, and quality of life
- Educate clients and families on home exercise programs and therapeutic strategies
- Collaborate with patients, families, and interdisciplinary healthcare teams to ensure coordinated, optimal care
- Document treatment plans and patient progress accurately using electronic health record systems, while maintaining current best practices
- Licensed Occupational Therapist in the state of Florida
- At least 1 year of experience in occupational therapy
- At least 1 year of experience working in a Home Health environment
- Reliable transportation and valid driver’s license
- Passionate, professional, and committed to excellent care
