Job summary

Lincare seeks a dedicated home health professional to perform patient follow-up, educate patients, and conduct assessments in line with physician orders. The role includes delivering enteral supplies and maintaining patient records with attention to regulatory standards.

Ideal candidates have experience in home health or nursing-related fields and strong communication skills to interface with doctors, referral sources, and patients.

Qualifications

• One year of experience in home health care, hospital, clinic, or nursing home strongly preferred.
• Experience/Education in Dietetics, Nutrition, Home Health, or Nursing strongly preferred.
• One year of experience as an LPN, DTR, or CNA preferred.

Responsibilities

• Provides patient education as part of patient care per policy.
• Performs complete and professional patient assessments per procedures.
• Maintains current knowledge of enteral home care techniques and enteral therapy concepts.
• Represents the company through professional appearance and patient care.
• Gathers information from referral sources to ensure insurance coverage.
• Follows up on paperwork to ensure payment for services rendered.
• Maintains patient records in compliance with accreditation standards.
• Drives to deliver enteral supplies to patients' home or business.
• Provide patient care, treatment, and service in a coordinated manner.
• Promote safe environments and proper feeding equipment use.
• Participate in ongoing education, in-services, and training to maintain competence.
