Job summary

Sanford Health in the heartland is seeking a Clinical Care Leader (CCL) to provide individualized nursing care in the home setting using the nursing process. You will collaborate with an inter-professional team to plan, implement, and evaluate care while documenting resident history, symptoms and medications.

The role requires an RN license with BSN, BLS, and a minimum of two years of clinical experience. You will delegate to and supervise unlicensed personnel and may take on leadership

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Minimum of two years’ clinical experience preferred.
• Unencumbered RN license with the State Board of Nursing.
• A valid driver’s license may be required depending on location.
• Basic Life Support (BLS) required.
• RAI process experience is preferred but not required.

Responsibilities

• Collaborates with inter-professional colleagues to plan, implement and evaluate care.
• Documents resident history, symptoms, medications and care provided in compliance with applicable standards/regulations.
• Utilizes technology to enhance quality resident care and collaborates with the inter-professional team.
• Effectively delegate to and supervise unlicensed personnel and assume nursing accountability for a resident assignment.
• Serve in leadership roles on department/organizational tasks forces and committees.
