Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team to provide compassionate patient care in a home setting. You will apply the nursing process, assess needs, and coordinate with a patient-centered care plan under the direction of the Director of Clinical Services.

Qualifications

• Current RN license for the state of practice.
• Current health certificate as required by facility/state regulations.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and documents nursing diagnoses.
• Document the plan of care with diagnosed outcomes and interventions.
• Performs interventions according to priorities, plan of care and hospital policies.
• Revise the plan of care based on evaluation and changes in medical plan of care.
• Uses clinical judgment in evaluation activities to meet patient care needs on assigned unit.
• Other Registered Nurse (RN) duties as assigned.
