Centria Healthcare is hiring dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide continuous 1:1 private duty nursing for a client in Wellston, MI.

This 24/7, high-acuity home care assignment requires medical expertise in managing ventilator support, feeding tubes, wound care, and ADLs, while collaborating with family and Centria’s clinical leadership.

Shifts are 12 hours (day or night), with full-time, part-time, and PRN options and weekly pay.

Qualifications

• Active RN or LPN license in Michigan.
• Home care or private duty experience preferred.
• Strong communication and critical thinking skills.

Responsibilities

• Provide continuous 1:1 skilled nursing care tailored to complex medical needs.
• Manage tracheostomy, ventilator support, feeding tubes, wound care, and meds.
• Monitor vital signs and track changes in condition daily.
• Respond to health status changes and emergencies promptly.
• Assist with ADLs and promote comfort, dignity, and mobility.
• Maintain accurate electronic charting and documentation.
• Collaborate with family, physicians, and Centria leadership to optimize outcomes.

Private Duty Nurse (RN/LPN) – 1:1 Home Care

Job Summary

Centria Healthcare is hiring dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide continuous, high-acuity 1:1 private duty nursing for a client in Wellston, MI.

This client is authorized for 24/7 one-on-one care, typical of catastrophic injury recovery (such as Motor Vehicle Accident/MVA care). You will play a critical role in providing comprehensive, skilled clinical care in a comfortable home environment while working closely with the client, their family, and an interdisciplinary care team.

Responsibilities

Provide continuous, high-acuity 1:1 skilled nursing care tailored to the client's complex medical needs.

Manage advanced clinical care needs, which may include tracheostomy, ventilator support, feeding tubes, wound care, and medication administration.

Perform thorough physical assessments, monitor daily vital signs, and track changes in condition.

Respond promptly and effectively to subtle changes in health status or clinical emergencies.

Assist with activities of daily living (ADLs) and promote patient comfort, dignity, and mobility.

Maintain accurate, detailed, and timely electronic charting and documentation.

Collaborate with family members, physicians, and Centria’s clinical leadership team to optimize outcomes.

Shifts: 12-Hour Shifts (Day Shifts & Night Shifts Available)

Coverage Need: 24/7 Continuous Care Authorization (Full-Time, Part-Time, and PRN opportunities)

Pay: Weekly pay + optional daily pay; competitive hourly rates based on experience and shift differentials.
