Qualifications

• Illinois RN or LPN license is required.
• Graduation from an approved nursing school is required.
• At least one year of RN experience and the ability to provide quality patient care.
• Current driver’s license and ability to drive between patient locations.
• Ability to meet charting deadlines and manage schedules.

Responsibilities

• Deliver high-quality, patient-centered in-home care.
• Collaborate with clinicians across behavioral health, home care, and home health.
• Document care and chart in EMR on schedule.
• Maintain compassionate, dignity-forward care.
