Home Healthcare, Hospice & Community Services is seeking a compassionate Home Care Nurse to serve Keene area patients in their homes. The role emphasizes relationship-building, safety, and independence, with nursing expertise to assess needs and coordinate care.

You will work within an interdisciplinary team to deliver personalized plans, educate families, and advocate for access to services and resources, ensuring high-quality, patient-centered care in home settings.

Qualifications

• Graduate of a state-licensed basic nursing program.
• Current New Hampshire RN license in good standing.
• Current CPR certification.
• One year of professional nursing experience or equivalent.
• Valid driver’s license and reliable access to an automobile.
• Ability to use EMR and protect patient information.
• Strong interpersonal skills and sensitivity with patients and families.

Responsibilities

• Perform admission and ongoing patient assessments (physical, psychosocial, environmental).
• Develop, implement, and update individualized care plans.
• Provide skilled nursing interventions within your scope of practice.
• Monitor patient conditions and communicate changes to the care team.
• Educate patients and caregivers on treatment, recovery, and symptom management.
• Promote independence and autonomy for patients and families.
• Coordinate care with case managers, physicians, therapists, and aides.
• Advocate for access to services, equipment, medications, and community resources.
• Maintain accurate clinical documentation using EMR and mobile tools.
• Participate in interdisciplinary meetings and quality improvement activities.
• Delegate tasks and supervise according to professional judgment and regulations.
• Maintain productivity with an average of 5–7 patient visits per day.
• Comply with safety, infection control, confidentiality, and regulatory requirements.
• Precept students and new staff as requested.
