You’ll perform assessments, manage medications, educate patients and families, and maintain accurate notes while working independently to support home‑bound patients across the region.

Qualifications

• Active New Jersey NP license
• DEA license
• Malpractice coverage
• Strong communication skills and professionalism
• Ability to work independently
• Proven NP experience in primary care, ER, clinic, or nursing home settings

Responsibilities

• Perform assessments, vitals, and physical exams
• Diagnose health conditions and illnesses
• Prescribe and manage medications
• Maintain accurate clinical notes and visit schedules
• Build trust and provide guidance to patients and families
• Educate on conditions, prevention, and overall health habits
