Job summary

AHN Healthcare@Home in Greensburg, PA is hiring a Home Health Medical Social Worker (MSW) for 32 hours per week within a clinician-led home health team.

You will conduct psychosocial assessments, develop care plans, connect families with resources, and document in EMR. Requires MSW with 1–2 years healthcare experience; licensure preferred. Travel to patient homes within territory; equal opportunity employer.

Qualifications

• Master’s in Social Work from graduate program accredited by the Council on Social Work
• 1–2 years post-graduate social work experience in a health care setting
• Licensed MSW preferred

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans addressing emotional, social, and environmental factors
• Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance
• Apply critical thinking and problem-solving skills to navigate complex situations, advocate for patient needs, and maintain accurate EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
