Job summary

Elara Caring seeks a Licensed Vocational / Practical Nurse to provide high-quality home health care across patient homes in Michigan. You will verify care plans, deliver nursing visits, and collaborate with RNs and physicians to ensure patient safety and comfort.

The role requires an active LVN/LPN license, at least 1 year of clinical experience, and up to 50% travel between patient homes. Hospice/home health experience is preferred, and reliable transportation is essential.

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational / Practical Nurse.
• 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment is preferred.
• 50% travel required.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes.

Responsibilities

• Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines.
• Incorporates patient care goals established in the Plan of Care into nursing visits.
• Communicates significant findings to the RN Case Manager, physician, and other personnel involved with patient care.
• Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician.
• Implements the plan for patient safety using patient, family, and community resources.
• Assists the patient in learning self-care techniques while delivering quality patient care.
• Suggests revisions to the plan of care in response to identified issues and notifies the RN Case Manager.
• Provides services required substantial nursing skills under the direction of a RN or physician.
• Collaborates with the RN Case Manager during normal business hours for guidance.
• Documents medication regimen and updates medication profile according to procedures.
• Treats patients and caregivers with care and respect.
• Aims to prevent avoidable hospitalizations and ER visits.
