Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our home health team. You will verify plans of care, deliver nursing visits, and collaborate with RN Case Managers and physicians to provide high-quality care in patients’ homes.

Ideal candidates hold an LVN/LPN license, have at least 1 year of clinical care experience, and are comfortable with 50% travel. We offer growth opportunities, comprehensive benefits, and a supportive team.

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• Current State License as a Licensed Vocational / Practical Nurse
• 1 year of experience in a clinical care setting
• Experience in hospice or home health environment preferred
• 50% travel required
• Valid driver's license and insurance and reliable transportation

Responsabilités

• Verifies the Plan of Care prior to each visit and follows RN delegation.
• Delivers care per physician orders and plan of care.
• Communicates findings to the RN Case Manager and physician.
• Implements safety plans and reports unsafe conditions.
• Documents medication regimens and updates patient profiles.
• Aims to prevent avoidable hospitalizations and ER visits.

Formation
