Job summary

Halcyon Home LLC in Arlington, TX is seeking a Home Health LVN PRN to support geriatric clients in Irving/Arlington/Eastern Tarrant County. Each visit delivers nursing care per plan of care, collaborates with RNs, and ensures compassionate patient support.

The ideal candidate is a graduate of an accredited LVN program, holds an active Texas license, with at least 1 year of clinical experience. Home health experience and Homecare Homebase familiarity are a plus; CPR certification is required.

Qualifications

• Graduate of an accredited school of vocational/practical nursing.
• Active state LVN license; multi-state license acceptable.
• Minimum 1 year of licensed practical nurse experience in a clinical care setting.
• Home health experience preferred.
• Homecare Homebase platform experience is a PLUS.

Responsibilities

• Under RN direction, identifies patient needs (physical, psychosocial, environmental) as evidenced by documentation.
• Plans and implements care with patient, caregiver(s) and RN; follows physician orders in the plan of care.
• Provides patient and caregiver education.
• Maintains infection control measures to protect staff and patient.
• Ensures continuity of care through quality patient care delivery.
• Communicates effectively with patient, family, team members and healthcare professionals; completes timely documentation.
• Monitors assigned cases for third‑party payor requirements.
• Participates in the Agency’s QAPI program and in‑service trainings.
• Demonstrates commitment, professional growth and competency.
• Promotes agency philosophy and policies to ensure quality of care.

About this Position

Halcyon Home is hiring a Home Health LVN PRN - Per Visit to support our growing geriatric home health population in Irving/Arlington/Eastern Tarrant Country, TX and surrounding areas. We offer competitive pay, and wonderful team support.

Essential Functions

• Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
• Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
• Provides patient and caregiver education.
• Provides care using infection control measures that protect both staff and patient.
• Assures the continuity of care through delivery of quality patient care.
• Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
• Monitors assigned cases to ensure compliance with requirements of third‑party payor.
• Participates in the Agency’s QAPI program.
• Participates in the Agency sponsored in‑service trainings.
• Demonstrates commitment, professional growth, and competency.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.

Requirements

Education: Graduate of an accredited school of vocational/practical nursing.

Licensure: Active, state license as a licensed vocational/practical nurse, or a multi‑state license issued by a Nurse Licensure

Experience: One year of experience as a licensed vocational/practical nurse in a clinical care setting required.

• Home health experience, preferred
• Homecare Homebase platform experience is a PLUS!

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification.

Transportation: Reliable transportation and valid auto liability insurance.
