Job summary

Be Healthy at Home in Texas is seeking a Licensed Vocational Nurse to provide in-home nursing care. You will assess patients and caregivers, educate them, and follow physician orders and agency policies. You must have at least 2 years of healthcare experience and excellent communication skills.

You will work under the direction of a registered nurse and comply with Medicare/Medicaid regulations. This role emphasizes teamwork, documentation, and adherence to infection control and HIPAA guidelines

Qualifications

• Licensed Vocational Nurse with current license.
• Minimum of 2 years' healthcare experience.
• Excellent oral and communication skills.

Responsibilities

• Demonstrate efficient teamwork with the staff.
• Demonstrate organizational and time management skills.
• Support quality improvement practices.
• Perform nursing procedures according to agency policy and procedures.
• Work under the direction of a RN.
• Monitor patient progress using observation, assessment, and evaluation skills.
• Educate patients and family members about disease, medications, POC, treatment options, and home care procedures.
• Report adverse findings to physician and RN.
• Follow Medicare and Medicaid regulations.
• Coordinate and monitor patient care and services.
• Comply with HIPAA regulations in and out of the office.
• Follow infection control policy in and out of the office.
• Document skilled visit according to Medicare guidelines.
• Maintain patient records according to policy and procedures.
• Participate in in-services, workshops, seminars, and self-study courses annually.
