Mission Healthcare is seeking a licensed LVN/LPN to provide skilled nursing care in patients' homes within Ventura County. You will work under the supervision of an RN or physician, monitor health status, administer medications, and educate families on care plans.

Qualifications include an active LVN/LPN license, CPR/BLS certification, and 1+ year of home health or med/surg experience. A valid driver’s license and reliable vehicle are required for travel to patient homes.

$38 to $45 per hour, depending on experience

• Educate patients and families on disease management and care plans.
• Document patient visits and maintain accurate medical records.
• Collaborate with interdisciplinary team to ensure quality patient care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program (LVN/LPN).
• Minimum 1 year of experience, preferably in home care or med/surg.
• Current state license and CPR/BLS certification.
• Strong communication and problem-solving skills.
• Ability to meet physical demands of the job.
• Valid driver's license and a reliable, insured vehicle.
