Job summary

Team Select Home Care is seeking a Licensed Vocational Nurse to provide skilled home nursing under the direction of an RN in patients’ homes. You will administer medications, follow plans of care, educate families, and document visits in an EMR system with confidentiality and HIPAA compliance.

The role requires IV/CPR credentials and a compassionate approach to pediatric and adult home care. Training and mentorship are offered to new graduates, with flexible scheduling including weekends and

Qualifications

• Graduate of an approved school of vocational nursing and currently licensed in the state.
• One year nursing experience preferred.
• Active CPR certification with practicum.
• IV certification by state mandate, if required by the state.
• Excellent verbal and written communication.

Responsibilities

• Furnishes skilled nursing care under the direction of an RN.
• Administers medications and treatments per plan of care.
• Educates families on home care techniques.
• Documents visits in EMR accurately.
• Takes on‑call duties, nights, weekends and holidays as assigned.
• Maintains confidentiality of PHI per HIPAA requirements.
