Job summary

Kona Medical Consulting and The Home Health Team is seeking a dedicated Home Health Field LVN to provide skilled nursing in patients' homes within the Dallas–Fort Worth metroplex. The ideal candidate is dependable, clinically confident, and enjoys independent work with strong team support.

Responsibilities include wound care, meds, vitals, and teaching; collaborate with RNs and physicians, document in EMR, and travel to patient homes as needed.

Qualifications

• Current Texas LVN license.
• 1+ year of nursing experience (Home Health preferred).
• Strong assessment, communication, and clinical skills.
• Reliable transportation, valid driver’s license, and auto insurance.
• Familiarity with EMR / mobile charting.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Perform wound care, medication administration, vitals, and disease teaching.
• Assist with care plan implementation and updates.
• Conduct patient assessments and report changes promptly.
• Collaborate with physicians, RNs, and team members for quality care.
• Maintain timely, accurate EMR documentation.
• Travel to patient homes within a service area.
