Job summary

Kona Medical Consulting and The Home Health Team are seeking a Home Health Field LVN to deliver skilled nursing care to patients in their homes in the Dallas–Fort Worth area. The ideal candidate is dependable, clinical in mindset, and enjoys autonomous work with strong team support.

Responsibilities include wound care, medication administration, vital sign monitoring, and teaching patients and families. Travel within an assigned service area is required.

Qualifications

• Current Texas LVN license is required.
• 1+ year nursing experience, Home Health preferred.
• Strong assessment and communication skills.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Perform wound care, meds administration, vitals, and teaching.
• Assist with care plan updates and implementation.
• Conduct patient assessments and report changes promptly.
• Collaborate with physicians and RNs to ensure quality care.
• Maintain accurate EMR documentation.
• Travel to patient homes within service area.
