Job summary

Cantex in Beaumont, TX seeks an LVN Clinical Coordinator to support clinical operations, care coordination, and documentation management in home health. You will work with the DCS, physicians, and the care team to track workflows, ensure timely follow-ups, and maintain HIPAA-compliant records.

Role requires strong communication, attention to detail, and the ability to prioritize in a fast-paced environment while maintaining regulatory compliance and high-quality patient care.

Qualifications

• Current, unrestricted LVN license required.
• Minimum 2 years in home health or similar setting.
• Strong knowledge of home health documentation and regs.
• Experience with HCHB, Forcura or similar systems.
• Ability to manage multiple tasks in a fast-paced environment.
• Excellent communication with clinicians and leadership.
• Knowledge of Medicare CoPs and home health requirements.
• Work independently while supporting a collaborative team.

Responsibilities

• Track and follow up on lab results and clinical findings.
• Identify overdue results and escalate to leadership.
• Communicate results and updates to physicians; document accurately.
• Monitor charts for follow-up actions and compliance.
• Triage clinical calls from field staff, patients, caregivers.
• Document communications and track resolution activities.
• Manage HCHB, Forcura, and document-management queues.
• Track orders, F2F documentation, and records requests.
• Coordinate follow-ups with clinicians, intake, QA, and physicians.
• Review Face-to-Face documentation for completeness.
