Job summary

Halcyon Home is hiring a Home Health LVN PRN to support our geriatric population in San Antonio, TX and surrounding areas. You will work under the RN in patients' homes and partnering communities.

A valid LVN license, CPR certification, and at least one year of clinical home health experience are required; Homecare Homebase experience is a PLUS.

Qualifications

Education: Graduate of an accredited school of vocational/practical nursing.
Licensure: Active, state license as a LVN/LPN, or multi-state license.
Experience: One year as a LVN in a clinical care setting required.
Home health experience preferred.
Homecare Homebase platform experience is a PLUS!

Responsibilities

Under RN direction, identifies patient needs (physical, psychosocial, environment) as evidenced by documentation.
Plans and implements care in partnership with patient, caregiver(s), and RN.
Monitors cases to ensure third-party payer requirements are met.
Participates in QAPI and agency trainings.
Provides patient and caregiver education and supports infection control.
Documents in a timely manner and ensures continuity of care.

Essential Functions:

Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
You will be expected to be present within our partnering communities and our clients independent residences.
Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
Monitors assigned cases to ensure compliance with requirements of third-party payor.
Participates in the Agency’s QAPI program.
Participates in the Agency sponsored in-service trainings.
Demonstrates commitment, professional growth, and competency.
Promotes the Agency philosophy and administrative policies to ensure quality of care.
Provides patient and caregiver education.
Provides care using infection control measures that protect both staff and patient.
Assures the continuity of care through delivery of quality patient care.
Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
