Job summary

Trinity Health Group is seeking a Licensed Vocational Nurse to provide practical nursing care in collaboration with RN case managers and supervisors. You will educate clients and families per the care plan and perform under supervision of RNs and Administrators.

The role includes both full-time and PRN positions in Texas and home health settings. Applicants must be a graduate of an approved LVN program with an active Texas LVN license and at least one year of LVN experience, preferably in home

Qualifications

• Graduate of an approved LVN nursing program.
• Active Texas LVN license to practice.
• Minimum of one year LVN experience, preferably in home care.
• Criminal background and required checks.
• Current CPR certification.
• Vehicle for client visits and lifting devices as needed.

Responsibilities

Job Summary

A Licensed Vocational Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates clients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RN’s, Administrator, and Supervising Nurse.

Qualifications

• A graduate of an approved LVN nursing school

• An active license to practice professional practical/vocational nursing in Texas.

• A minimum of one (1) year of experience as a LVN, preferably in a home care setting.

• Will have a criminal background check and any other required checks.

• Will have current CPR certification.

• Vehicle to do routine client visits, possible transfer/lifting devices, medical supplies
