Job summary

Benedictine Living is seeking a Licensed Practical Nurse to provide care for a group of home health patients/residents under supervision of a Registered Nurse, following established policies and standards of practice.

You will collect data, participate in care plans, administer medications, educate families, document outcomes, and assist with discharge planning. A valid LPN license and BLS are required, along with a Class A driver's license and a clean driving record.

Qualifications

Current state licensure as a Practical Nurse in good standing.
BLS Certification is required.
Valid Class A Driver’s License with an acceptable driving record.

Responsibilities

Perform initial data collection on patients/residents and follow up as per policy.
Support person-driven care plans developed by the Registered Nurse within policies and procedures.
Administer medications and document patient responses.
Educate patients/residents and families per plan and document responses.
Document in accordance with current standards.
Plan for patient discharge with RN and discharge planners.
