You will perform initial data collection and participate with RNs to implement person-driven care plans, using Relationship Based Care principles.

Qualifications

• Current State licensure as a Practical Nurse in good standing.
• Must be able to read, write and communicate in English.
• Clinical experience preferred.

Responsibilities

• Collaborate with RN to implement person-driven care plans.
