Job summary

VitalCaring Group is seeking a Licensed Practical Nurse (LPN/LVN) for Home Health to deliver one-on-one care in patients' homes. You will implement care plans, administer treatments, and educate patients and families to support recovery and independence.

As part of a collaborative clinical team, you’ll work under an RN supervision, maintain accurate documentation in the EMR, and adapt to a flexible field-based schedule that prioritizes patient comfort and outcomes.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care under the supervision of the RN.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care by maintaining consistent communication across the care team.
