Join Our Team: Home Health LPN Intake Specialist (Weekends)
Department: Home Health.
Schedule: Saturday and Sunday, 8:00 AM–4:30 PM.
Location: VNA of Hanover & Spring Grove.

The VNA of Hanover & Spring Grove is seeking a compassionate and organized Licensed Practical Nurse (LPN) Intake Specialist to join our Home Health team. This part-time weekend role is essential in coordinating patient intake and referrals, ensuring a smooth and timely start of care for those in need of home-based services.

What You’ll Do:
Referral Intake & Coordination:
  • Receive and process referrals from hospitals and physicians.
  • Enter patient demographics and clinical information into our EMR.
  • Obtain and review required medical documentation and physician orders.
  • Collaborate with scheduling, insurance, and billing teams.
  • Triage incoming patient concerns and coordinate care for Home Health or Hospice.
  • Assign appropriate staff to initiate and support care delivery.

Patient & Family Communication:
  • Serve as the point of contact for patients, families, and referral partners.
  • Answer inquiries with professionalism, clarity, and empathy.
  • Support the patient transition process with education and guidance.

Nursing & Team Support:
  • Assist RNs in care planning and provide nursing care within LPN scope.
  • Document clinical observations and support continuity of care.
  • Contribute to a collaborative, mission-driven team culture.

Quality & Compliance:
  • Ensure compliance with Medicare, HIPAA, and agency standards.
  • Participate in quality improvement and staff training initiatives.

What You’ll Need:
  • Current LPN license in Pennsylvania.
  • Strong organizational and communication skills.
  • EMR experience preferred.
  • Valid driver’s license, reliable transportation, and insurance.
  • Weekend availability (Saturday & Sunday, 8 AM–4:30 PM).
